Condition of this registration relating to carrying out this regulated activity
Within 56 days of the conditions taking effect, and thereafter on the third Wednesday of every month, the registered provider must provide an updated report of the actions taken to address the risk to service users in relation to the conditions in this notice or highlighted by the governance system. This should
include detailed evidence of how you will achieve this and monitor ongoing compliance. The report must include evidence demonstrating completion for any action deemed completed at the point of submission.
Within 28 days of the conditions taking effect, the registered provider must provide an action plan and report detailing the action taken in accordance with the conditions in this notice. The action plan and report must include details of action owners, how actions will be assessed and monitored and the expected timelines for actions to be completed.
Within 28 days of the conditions taking effect, the registered provider must implement an effective governance system. This must assess, monitor, and drive improvement in the quality and safety of the services provided, including the quality of the experience for service users in line with the regulations. This governance system must ensure:
a. Risks in services are appropriately recorded, assessed, escalated to the trust’s board where required, and regularly reviewed.
b. Progress against action plans is monitored to improve the quality and safety of services and appropriate action is taken without delay where progress is not achieved as expected.
c. An effective system to identify and report incidents including the severity of harm. The system must ensure incidents are appropriately reported to internal and external systems within appropriate timescales.
The system must ensure incidents are effectively reviewed, lessons and
actions are identified and shared with staff.
d. There are effective quality assurance systems in place to support the delivery of safe and quality care.
e. Feedback from staff is used to drive improvements to the quality and safety of services, and once improvements are identified they are made without delay.
f. Staff are able to report service user safety concerns without fear of reprisal, retribution or detriment using internal routes and in line with policies and procedures.
g. Feedback from external bodies such as royal colleges and other bodies who provide best practice guidance is sought and acted upon.
