Manager application: Continue my registration under a new provider

This form is for registered managers who are already registered with us but whose locations are being taken over by a new provider.

In these circumstances, this form allows you to:

• deregister as the registered manager from their current provider, and
• register with us as a registered manager with a new provider.

If you are moving your job to a new provider and location, you must cancel your manager registration then apply to register as a new manager.
